
Recognizing the limitations of traditional, hierarchical healthcare models, Puentes was founded on the belief that care must be interdisciplinary and collaborative. We decentralize the role of the physician, empowering health advocates (promotoras de salud), nurses, nurse practitioners, behavioral health consultants, social workers, students, and doctors to work as a team in serving our community.
We are proud to be supported by an extensive network of volunteer physicians from across the city, representing a wide range of specialties. These volunteers provide direct care, and help us extend our reach far beyond what a traditional clinic might offer.
This unique model, led by nurses, and supported by a wide array of volunteers, is part of what makes Puentes de Salud both innovative and deeply responsive to community needs.


Our clinic is nurse-managed, led by two Certified Registered Nurse Practitioners (CRNPs) who serve as Co-Medical Directors. Alongside them, our Community Nurse Liaison (CNL) plays a hybrid role that blends direct patient care, advocacy, and organizational leadership. Together, this core nurse team provides bilingual, culturally responsive, and high-quality primary care tailored to the needs of Philadelphia’s Latine immigrant community..
Our nurses are clinicians and they are strategic leaders as well. They guide care delivery, shape policy, and ensure that our model remains rooted in dignity, equity, and community voice.
